In compliance with the provisions of Personal Information Protection Law in Japan, the Company shall handle requests for disclosure, etc.of Personal Information from any costomer in adequate manners as indicated below.

Please output the “Request form for Personal Information Disclosure (pdf)” or the “Request form for Correction and Usage Termination (pdf)” for submission. |

Please FAX your request for the “Request form for Personal Information Disclosure” or the “Request form for Correction and Usage Termination” along with your name, FAX number and telephone number to Yamato Holdings Co., Ltd., or contact our customer service center. The form(s) will be sent to you later by FAX. |

Personal Identification |
| |
|
Based on the document you have sent in, the Company will confirm your identity as the principle. Our customer service center may call you by telephone to inquire if you have actually made the request. |

(3) |
In-house Search |
| |
|
Based on your request information sent in, the Company will initiate an in-house search. While the Company will do its best in the search, there may be some data which cannot be extracted due to difficulties in data retrieval. |

(4) |
Reporting the Results |
| |
|
Within one month after receipt of your request, the Company will send you a report of the search results. If your documents are not complete, however, the Company may require more time in reporting. |

| 2. |
Charges |
| |
For your request for personal information disclosure, the Company will charge 1,000 yen (including the consumption tax) for handling the search. The charge will be made upon delivering you the report. |
